About Health Coaches
The Health Coaches team focuses on promoting sustainable lifestyle changes through behavioral coaching, emotional support, and personalized guidance. The goal is to enhance members’ quality of life while reducing overall healthcare costs through proactive health management.

Key Purpose
The Health Coach (Social Worker) – Independent Contractor is responsible for guiding members through structured coaching programs, ensuring that both program objectives and individual health goals are successfully achieved. The role emphasizes long-term lifestyle improvement and personalized support.

Key Responsibilities

  • Deliver effective coaching using a range of techniques to help members achieve program and personal goals
  • Build strong, supportive relationships to drive positive outcomes
  • Plan and manage coaching schedules to maximize participation and completion rates
  • Assist members with benefit navigation, referrals, and activation of available services
  • Provide education on health, lifestyle management, and chronic disease prevention
  • Support members in managing chronic conditions to reduce relapse, hospital admissions, and disease progression
  • Accurately track, record, and report all coaching interactions and progress
  • Meet performance targets including productivity, quality, and customer satisfaction
  • Engage eligible members and encourage enrollment into coaching programs
  • Stay updated with industry trends and healthcare developments
  • Maintain professional and effective communication in all interactions

Qualifications and Experience

  • Clinical qualification accredited with a recognized health professions body
  • Knowledge of healthcare products, benefits, and policy structures
  • Coaching experience is advantageous
  • Strong communication skills, both written and verbal
  • Proficiency in MS Excel, Word, and PowerPoint
