About the Program

Main Job Purpose
The Client Liaison Officer (CLO) supports clients at the Ivan Toms Centre, its mobile units, and the Call4Care line. The role provides direct support to the Project Manager to ensure client retention, referral, and linkage for ART, PrEP, and general healthcare services are fully implemented according to protocol.

Key Performance Areas

Supporting Retention in Care

  • Ensure high-quality service delivery and maintain supportive relationships with clients to promote retention in care.
  • Trace and engage clients who are lost to follow-up, late for appointments, or high-risk, as directed by the Project Manager or Senior Clinical Advisor.
  • Identify clients at risk of not initiating treatment or defaulting, and facilitate support to address these gaps.
  • Support chronic dispensing unit activities in coordination with the Project Manager and Senior Clinical Advisor.

Client Engagement and Linkage

  • Act as a primary contact for clients of the Ivan Toms Centre, community mobile clinics, and the Call4Care line.
  • Welcome clients professionally and ensure linkage to appropriate service providers according to protocol.
  • Support the Call4Care line during business hours to facilitate timely client access to services.

Testing and Education

  • Conduct HIV counselling and testing and/or distribute HIV Self-Screening kits in line with DOH and Ivan Toms Centre guidelines.
  • Provide detailed client counselling, including couples counselling, index testing, and risk assessments as required.
  • Manage cases for newly diagnosed clients or those assigned by clinical staff, facilitating ART initiation and viral load suppression.
  • Deliver comprehensive education on HIV prevention, treatment, STI awareness, and PrEP.

Maintenance of Records and Reporting

  • Maintain accurate communication records and provide daily/weekly reports on client engagement, progress, and outcomes.
  • Complete all required DOH, Ivan Toms Centre, and Anova data collection and reporting tools.
  • Maintain detailed case management records for each client.
  • Perform administrative duties or other tasks as requested by the Retention Coordinator or Project Manager.
