About the Program

Main Purpose of the Job
The Clinical Nurse Practitioner delivers high-quality, nurse-led HIV and primary healthcare services to men who have sex with men (MSM) and other key populations through the Ivan Toms Centre, mobile units, and other DOH sites. This role provides integrated clinical care across the HIV prevention and treatment cascade, ensures client retention and viral suppression, adheres to national clinical guidelines, and contributes to client-centred, stigma-free healthcare services.

Roles and Responsibilities

Clinical Service Delivery, Cohort Management, and Retention Support

  • Provide comprehensive clinical services at the Ivan Toms Centre, mobile units, or other clinic sites, including HTS, ART, PrEP, PEP, STI and TB screening, and STI treatment.
  • Deliver enhanced clinical services within scope, including prescription of medication, management of NCDs, mental health conditions, and other medical needs.
  • Conduct clinical services under supervision from the Senior Clinical Advisor when required.
  • Ensure all clinical activities comply with licensed scope of practice, organizational SOPs, and National Department of Health guidelines.
  • Monitor and manage allocated client cohorts to ensure viral suppression, PrEP continuation, and other clinical needs are addressed.
  • Achieve project performance targets through effective clinical service delivery.

Clinical Oversight and Mentorship

  • Provide clinical guidance and oversight to professional nurses in the absence of the Senior Clinical Advisor or when requested.
  • Serve as an initial referral point for clinical guidance within scope, facilitating consults with the Senior Clinical Advisor as needed.
  • Manage clinical referrals and linkage to tertiary institutions and other healthcare providers under established scope and supervision.

Quality Assurance and Routine Monitoring

  • Implement daily quality assurance to ensure accurate reporting and clinical service delivery.
  • Oversee clinical quality of mobile and facility-based teams, including non-clinical staff.
  • Conduct clinical QA procedures, including rapid test continuous quality improvement, per project, organizational, and National Department of Health guidelines.
  • Maintain up-to-date proficiency with guidelines, licenses, clinical practice standards, and SOPs.
  • Manage clinical practice spaces, adhering to project and site procedures, health and safety, and infection control policies.
  • Maintain operational records, perform routine inventories, and reconcile clinical materials, equipment, and medications.

Project Support and Administration

  • Complete programme reporting and data management, including updating clinical records, registers, and routine performance reports.
  • Support training and capacity building of clinical and non-clinical staff and other stakeholders.
  • Provide additional administrative, clinical, and programme support as requested, including duties delegated by managers.
